Category C Software Questionnaire

The Category C software questionnaire has been conducted every December since 2005. We would appreciate your cooperation.

 

1. Click on the 2023 Category C Software Continuation Application Questionnaire.
You will be asked to sign in to access Microsoft OneDrive. Enter your university account.

２．Enter the account name and password for ADFS authentication and click "OK".

Windows/ADFS password (same as Scomb)
※@sic is required.

 

３．click "next"

 

You have successfully accessed OneDrive.
Department/major name folders are displayed(red frame).

 

The following items are included in the department/major name folder.

  • Questionnaire for Continuation of Category C Software Application （excel file）
    Please answer whether or not you want to continue.
  •  Application form for new, change, or abolition of Category C software
     The URL of the application form is described. Please answer if there is new/changed/discontinued software.

    We will notify you only if there is a software license or price change from the vendor.

 

１．Open the department/major name folder and open the continuation application form.xlsx file.

 

 

２．After checking the software name on the left side of the window, please check the contents of the red frame in order.

 

①continuation answer

②Modified if the person in charge of the software changes

③Course name (if there are multiple courses, one course will be the representative)

④Software usage period

⑤information to information system section

 

３．We will contact you if there is no entry by the deadline.
